<p>The origin of this book lies on the awareness that we have to do more for identifying unmet needs and improving a tailor-made treatment for each <strong>newborn</strong>.</p>
<p>After a large introduction on <strong>developmental pharmacology</strong>, this book analyses in detail new advances on <strong>antibiotics</strong> (aminoglycosides), <strong>antifungals</strong>, <strong>antivirals</strong> (CMV, RSV, HIV), <strong>analgosedatives</strong>, <strong>steroids</strong>, <strong>caffeine</strong>, and <strong>cardiovascular drugs</strong>. An updated <strong>pharmacologic treatment</strong> of <strong>respiratory diseases</strong> (respiratory distress syndrome, meconium aspiration syndrome, chronic lung disease, persistent pulmonary hypertension) is presented, as well as for <strong>persistent patent ductus arteriosus</strong> and <strong>retinopathy of prematurity</strong>. Finally, <strong>neuroprotection</strong> is considered, with special emphasis on <strong>antioxidants</strong> and <strong>melatonin</strong>.</p>
<p>A special part is devoted to <strong>drug-induced renal toxicity and liver toxicity</strong> and completes the volume, also quoting the so-called &#8216;<strong>-omics</strong>&#8216;<strong> sciences</strong> such as genomics and metabolomics.</p>

L’attenzione si focalizza sul ruolo della donna, sull’<strong>accoglienza del neonato</strong>, sulla percezione sociale dell’<strong>allattamento al seno</strong>, e su terreni meno esplorati quali la <strong>fertilità</strong> e l’<strong>adozione</strong>. Ne emerge una <strong>rappresentazione culturale della nascita</strong> divisa tra antiche <strong>usanze</strong> e una visione sempre più scientifica della <strong>gravidanza</strong> e del <strong>parto</strong>.<br />
Questo saggio contribuisce a un approfondimento delle <strong>illness narratives</strong>, favorendo la ricerca di una convergenza tra il linguaggio medico settoriale e il linguaggio dell’esperienza culturale, perché la <strong>evidence based medicine</strong> non si contrapponga alla <strong>narrative based medicine</strong>, ma i due linguaggi convergano verso una complementarità che possa rafforzare l’alleanza tra medico e paziente.</p>

<p>The leading elements in this volume are the <strong>cultural representation of birth</strong> and the forms through which its narration and representation develop in the <strong>figurative arts</strong>, through <strong>historical references</strong>, <strong>mythological tales</strong> and <strong>legends</strong>, <strong>traditions</strong>, <strong>customs</strong> and <strong>habits</strong>. The influence of <strong>myth</strong>, <strong>language</strong> and <strong>artistic expression</strong> on our cultural representation of <strong>procreation</strong> is manifest, and this way of “narrating” birth resists even today, although it comes into conflict with a more scientific vision of <strong>pregnancy</strong> and <strong>childbirth</strong>.<br />
With this book we believe we have contributed to an in-depth examination of <strong>illness narratives</strong>, thus favouring the search for a convergence between medical language in the sector and the language of cultural experience so that <strong>evidence-based medicine</strong> does not clash with <strong>narrative-based medicine</strong>, but that the two languages come together towards a reciprocity that will strengthen the alliance between physician and patient.</p>

<p>Questo saggio scientifico, dedicato ai <strong>gemelli</strong>, si muove in due direzioni: approfondire gli aspetti biologici e clinici più attuali collegati con il <strong>concepimento</strong> e la <strong>nascita</strong> di gemelli; favorire un approccio multidisciplinare alla gestione delle <strong>gravidanze gemellari e multiple</strong>, cercando sinergie tra operatori di area ostetrico-ginecologica e pediatrico-neonatologica. L’attualità del tema è legata anche all’aumento del numero dei gemelli (oggi intorno al 3%) e al loro maggior <strong>rischio di prematurità e malnutrizione fetale</strong>. Sono affrontati nei vari capitoli <strong>aspetti genetici e malformativi</strong>, temi inerenti la <strong>gestione prenatale e perinatale delle gravidanze gemellari</strong> anche in rapporto alle pratiche di <strong>riproduzione medicalmente assistita</strong>, i problemi dei neonati sia alla nascita o in UTIN, che a distanza, compresa l’eventuale comparsa di esiti neuropsichici. Sono state approfondite le peculiarità dei <strong>gemelli monozigotici</strong>, gemelli con una condivisione completa del patrimonio genetico ereditato, ma che costituiscono la categoria a più alto tasso di patologie e complicanze, dalle <strong>malformazioni congenite</strong> ai <strong>danni emodinamici e cardiovascolari</strong>. I tassi di <strong>complicanze</strong> raggiungono il massimo se i gemelli, oltre ad essere <strong>monozigotici</strong>, sono anche <strong>monocorionici</strong> e <strong>monoamniotici</strong>, se condividono cioè anche l’<strong>ambiente intrauterino</strong>.</p>
